Abstract

Introduction: Diabetes mellitus is a chronic disease in which insulin production is low or absent, generating a metabolic imbalance that can compromise several organs. Type 2 diabetes mellitus (T2DM) is the most common type diagnosed in adults and, being largely silent, is often only treated after complications appear. Objective: To describe conventional treatment of T2DM in Brazil and in the United States and the current status of stem cell (SC) therapy, considering evidence, safety, and health regulation. Design: Narrative review of publications from 2000 to 2026 retrieved from PubMed, SciELO, Google Scholar and Consensus, selected by predefined inclusion and exclusion criteria; 30 references were included. Main results: pharmacological treatment remains the therapeutic backbone of T2DM in both Brazil and the United States, with growing regulatory convergence between ANVISA and the FDA regarding newer drug classes; stem cell therapy shows promising results and a favorable safety profile in controlled clinical trials. Conclusion: stem cell therapy remains without regulatory approval for routine use in either country, reinforcing its experimental nature.
